I wish I had answers instead of hopes. I know nothing about how Colorado handles Medicaid (given the unemployment situation). I've been privileged to have decent health care through professional employment, but I am aware of that privilege. Did CO set up ACA exchanges with subsidies? Again, I'm sure you've tried everything. Just hoping something comes through and Trevor can get hired to a better situation. Definitely go with the appeal even if he doesn't want to go back there. They have the burden of proof,  and winning an appeal may give ground to get a settlement. That would help some. I'm going to keep hoping for you.
