I called my mom after work (from work) to see if she thought I should go see Grandpa one more time, and while I was talking to her...she got that the call that he had passed.
He isn't in pain anymore. He's with Great Grandma (his mother).

At least he will be comfortable ... if he's coherent, now is the time to talk with him, be there to comfort him when the time comes. My father was terminal for a couple of years, and stayed with me under 24 hour nurse care. When he was close, he told me he wanted to die in his own home. He called me one morning so I went to see him, he said today is going to be the day, I said sure as he told me that everyday. I left as I have serious health problems of my own, and had to be at the hospital, he was okay when I returned later so I went downstairs to feed his cats, when I returned he was dead.

Well, according to my mom, they gave Grandpa his last round of blood clot meds, made him comfortable...and just started pain meds aka "comfort meds"...and that's all they can do.
