yes, almost every Onsen (and private swimming pool) does not allow person with heavy tattoos to enter. though small tattoos which may not look "offensive" are supposedly ok, but heavy tattoos like sleeve ink and large shoulder ink are seen as offensive to them.
therefore i don't go Onsen and private swimming pool. you see many tattooed guys and gals on public beach , incidentally.
