Actually that is fine. Though you do not have to add the testimonial to the end of it. Just make sure you get it notarized. You can also look it up on line too. I actually had to do two seperate Powers of Attorney for my parents. One for the estate decisions, and one for medical. You might want to check your state to make sure only one is required.

You dont need to state all that. The witness and you will have to sign it at the bottom but do not need to be in the statement. As this is a General power of attorney you do need to check with your state to see if this or a living will is needed. Some require one or both. This might also fall under a special power of attorney. I wish I could help more but I just go by more or less what I have gathered being in the military.
