A bit extreme, but all in all, it doesn't seem much more extreme than electroshock therapy. Get the surgery and you're done with the worst of it. With electroshock, you have to keep going back every few weeks because it only works for a little while. Most of the anti-depressants we have only work for about a third of people at the most, and a lot of them are only slightly better than the rates you see with placebos. Obviously not a go-to solution, but some people don't respond to the vast majority of treatments we have, and if their case is severe, it's a lot better than waiting around for them to off themselves.
