Well, I think you're sets look great. Except for the smoking, your my type of girl, so I'm probably of course, bias.
First, I'm all for being healthy. I need to eat better, exercise more, and could lose 20 lbs myself. So, if losing 15 lbs is healthy for *you*, then I agree. As for healthy, clearing up the black lungs might be better.
Second, boob job ? I instantly say no. I'm more of an ass man though. I like the toned, athletic look better. Yes, big boobs look good. However, the smaller, firmer ones (like yours?) stay put better. All I really need is a handful anyways. My gf in college had a huge chest (natural). She always had issues with running, jogging, jump (think concerts and stuff). She need support while sleeping too. I remember massive sports bras and "uni-boob" conversations. I prefer a girl that challenge me both mentally and physically.
And if you'd want to get a boob job ... I'd say ensure you are happy with the reset of your health and/or weight first. Weight changes are easier to change if one dislikes them.
