Step 5) Submit requisite items for re-instatement:

  • Full name
  • Date of Birth
  • Address listed on license
  • Current Address
  • Driver License Number
  • Copy of Birth Certificate
  • Copy of Social Security Card
  • Copy of State ID
  • Copy of Insurance Verification
  • Money Order in the amount of $175.00 for re-instatement
  • Money Order in the amount of $10.00 for replacement license
  • Letter requesting re-instatement
  • Pre-paid overnite mailing package for return of replacement license


Sent today, set for arrival on Monday before 12PM. Return should be by Tuesday at 12PM if I'm lucky.

Total cost: $214.10
