I think the best advice you can receive has already been said by the folks above me. I'm pulling for you to get out of this rut by the company. You're smart as hell and graduating from where you went is no small feat!
However, don't make the same mistake I made w/ my time @ Bear Stearns: I prioritized my time and made them 1st before my personal life.

You can do it. Everyone hits this at some point in their career. I completely agree with the advice of asking for help, making sure you get detailed information from them about why your performance is unsatisfactory and addressing those issues one by one, and making work your priority right now. Hang in there, I know you can work through this.

I think I have a release to go back to work, however the Exempla doctor I saw today for the workmans comp decided that I can't drive even though my neurologist said I can. Stupid doctors.
All things considered though, the last week has been pretty good to me.

Well, sure, the workman's comp doc wants to cover your work's ass. They don't want any liability with you going back "too soon." Workman's comp sucks in many ways. I hope you don't have any other trouble with them - one of the secretaries at work has had all kinds of hell with them.
