I geuss the nitrogen truck was told not over 7 MPA and he went to 9 MPA and it expanded to 15 MPA. no one heard him say 9 MPA cause he had a shitty radio. and bad timing cause we were lifting pipe heavy still or close to transition so it should have moved very slowly if it did begin to rise. way before he hit 15 MPA anyone who's ran any sort of pump or pressure truck should know you shut down. you dont go over your max you fucken bleed off. we pumped nitrogen down the tubing but it was plugged in sand and when our driller lifted the tubing it helped that plug let go and that's what blasted the pipe up so fast into the derrick like that. good thing the snubber was fast on his slipps.. I was under the working floor and thier was a tool hand right on the floor when it happened.. driller was also on his pad.. could have been a little messy I think with that heavy tubing wipping out of the hole.
